TITLE REQUEST FORM
In order for you to transfer your vehicle registration to Pennsylvania from your prior state of residence, the Pennsylvania Department of Transportation requires you to submit the original title. This form is to be used only when your vehicle has a lien or is leased.
Please complete the fields below to authorize Fazio Tag Service, Inc., to request your title from the lien holder or leasing company for the purpose of obtaining a new Pennsylvania registration and title.
Upon your request, Fazio Tag Service will fax a letter to your lien holder or leasing company within 24 hours requesting the title be sent to our office to complete the transaction. It typically takes most lien holders and leasing companies approximately two weeks to send the title. Fazio Tag Service will call you as soon as we receive the title. Once your title is received by Fazio Tag Service we request that you complete the transaction within thirty days. It is often helpful for you to contact the lien holder or leasing company 48 hours after we make the request to be sure the proper department has the request.
